• It is presumed that the specimen belongs to the patients named or identified in the test requisition form.

• Results pertain to the specimen submitted and all the results are dependant on the quality of specimen received in lab.

• A test requested might not be performed for the following reasons :

1. Specimen quantity insufficient (inadequate collection/spillage during transit).

2. Specimen quality is unacceptable (Hemolysis/Clotted/Lipemic)

3. Incorrect specimen type.

4. Test cancelled before processing either on request of patient or Doctor of incorrect test code, test name or specimen received.

• Partial reproduction of this report is not permitted.

• All lab Reports / related information are subject to clinical interpretation through qualified Medical professionals.

• Lab Results must be interpreted with clinical judgement, in conjunct with clinical presentation, history and other relevant investigations.

• Test results depend upon quality of sample as well as assay procedures and may very from lab to tab and also from time to time for the same parameters for the 

same patient

• In case of any unexpected abnormality in the lab results, A repeat investigation may be entertained within 72 hrs from the time of testing on request of clients/Doctors.

• Neither My Sample Pathology nor its Propritor/Employees/Representatives assume any liability, Responsibility for any loss or damage that may be incurred by any 

person Including the patient, as a results of assuming the meaning or contents of the reports.

• My Sample Pathology shall not be liable to any subjective, Interpretative litigations pertaining to its tested results.

• There may be circumstances beyond our control that might delay test results.

• This Report is not valid for Medico-Legal Purpose

• My Sample Pathology outsources certain test to other lab for providing a wider test menu to its clients.

• Financial / monetary claims are subject to approval from its management and shall not exceed the stipulated test cost under any circumstances.

• Claims if any, are subject to the jurisdiction of Unnao, U.P. only.
